LEAP deeply understands the need to have someone on your team who knows your loved one and knows the importance of each decision you make. Maybe you have all the information you need, but want someone in your corner to be a part of important meetings, milestones, or transitions, simply to give you piece of mind and expand your circle of support. We can

  • Attendance and planning for PCP meetings

  • Support for TY year planning meetings with CCS

  • Navigating and communicating with state and local agencies for eligibility for services

  • Communication with case managers and service providers

  • 1:1 support meetings as often as you need to review your plans, thoughts, fears, and desires, whether it is to reach a goal or simply check to see if you are on the right track
